This link is a story about a US fighter that dropped live bombs around a manned range tower here in Aussie, because he stuffed up. He was suppose to drop them 2 km further down his vector. That was 2005.

I visited this range in 1996 and it had happened a few years earlier as well, with dummy bombs, and another US fighter, but still left craters around the tower.

Seems it was the RAF air traffic controllers at Lakenheath who fcuked up on that one. That's the way it reads to me.....pilot just went where he was instructed & then was not given further instructions after the handover.
